Post-exertional malaise (PEM) is a disproportionate worsening of symptoms after physical or mental exertion, even minimal. It is a hallmark symptom of Long COVID and chronic fatigue syndrome (ME/CFS). PEM can occur 24 to 72 hours after exertion and last several days.
